Case Management ACRA
provides case management services for its clients. Beginning in January
2003, there was a severe shortage of qualified case managers in parts
of the Triangle due to inadequate reimbursement by Medicaid. Many of
our residents had to do without case management services, and were left
to try to negotiate the complexities of managing their disease by themselves.
ACRA received approval from the state's AIDS Care Branch and a (3-year)
grant, enabling us to hire a full time case manager. Case management
services are initially being focused on ACRA's own residents.
On July 1, 2003, ACRA began offering case management services to HIV-positive
individuals in Durham and Orange counties who are not ACRA residents
or who are non-Medicaid eligible. Although ACRA began offering case
management services in January 2003, the main focus of our work remains
providing diverse housing options and compassionate care for individuals
and families living with HIV/AIDS. ACRA is considered
a leader in HIV/AIDS housing in the Triangle
and in North
Carolina.
Case management services are made available to HIV-positive individuals
through Medicaid and Ryan White funding for the purpose of assisting
them in accessing services in the community. The case manager, who
is certified by the AIDS Care Unit of the state, links clients directly
to agencies that can meet their health, housing and basic care needs.
Those qualifying for Medicaid are supported through this fund, while
non-Medicaid eligible clients are supported by the Consortium through
Ryan White Care funding. All in all, the full HIV community may be
serviced if need exists. ACRA is licensed by the State of North Carolina
to provide case management services in both Durham and Orange counties.
